Overview of NPPC molecular targets
C-peptide, also known as connecting peptide, is secreted by pancreatic β cells and has a common precursor, proinsulin, with insulin. It is a 31-amino acid polypeptide that connects the A chain and B chain of insulin in the proinsulin molecule. C-peptide has been shown to bind to G protein-coupled receptors on the surface of many types of cells (such as neurons, endothelial cells, fibroblasts, and renal tubules) at nanomolar concentrations. C-peptide exerts vascular anti-inflammatory effects by downregulating adhesion molecules on the surface of endothelial cells, reducing the production of inflammatory cytokines, and protecting vascular smooth muscle cells from harmful insulin effects. In animals with C-peptide-deficient (type 1 model) nephropathy, administration of C-peptide can improve renal function and structure, reduce urinary albumin excretion, and prevent or reduce diabetes-induced glomerular changes due to mesangial matrix expansion.
