Function
Uromodulin (UMOD), also known as Tamm-Horsfall protein (THP), encoded by the UMOD gene in humans, is the most abundant protein excreted in normal urine. It is produced exclusively by epithelial cells lining the thick ascending limb (TAL) of the loop of Henle and the early distal convoluted tubule of the kidney. Uromodulin is synthesized as a glycosylphosphatidylinositol (GPI)-anchored glycoprotein on the apical (luminal) plasma membrane; the ectodomain is then proteolytically cleaved by proteases such as hepsin and shed into the tubular lumen, where it polymerizes into a high-molecular-weight filamentous meshwork. Urinary uromodulin binds mannose-sensitive pili of uropathogenic E. coli, trapping bacteria and defending against urinary tract infection; it also inhibits calcium crystallization, modulates NKCC2 and ROMK sodium/potassium transport in the TAL, and downregulates renal inflammation. Mutations that impair UMOD maturation cause familial juvenile hyperuricemic nephropathy.
